Admission Requirement

• The applicant must be at least 17 years old (with parent/guardian permission) 

• The applicant must provide the school with an official high school diploma, transcript, or GED.

• The applicant must submit original immunization record.

• The applicant must have a passing score on the entrance test. The score of 17 or above is required to enter a program at Phlebotomy Career Training Academy. The Louisiana Proprietary School Commission has recommended that all proprietary schools utilize a standardized admissions test as part of the admissions criteria.

• The applicant must agree and sign the terms of the Enrollment Contract/Payment Plan Contract.

• The applicant must pay $150 non-refundable registration fee plus tuition on the day of enrollment. (Pay in Full)

• The applicant must pay $150 non-refundable registration fee plus $500 of the tuition on the day of enrollment. (Payment Plan)​​.
